ENTRY PROCEDURES • Complete application and mail original packet along with payment to: City of Pembroke Pines Fletcher Art and Cultural Center 7960 Johnson Street Pembroke Pines, Florida 33024 • Submit 3 Photos/slides of your work and 1 photo of your outdoor display labeled with your name. • Musicians must send 1 photo of your outdoor display and 1 CD or cassette of your music. • Enclose a self-addressed stamped envelope for the return of your slides/CD or cassette. • Please use proper postage. • Submit 2 checks, one for Jury Fee and one for Booth Fee:
