If you are interested in the Adult Day Care
program, please contact the Accounting Clerk – Contract Specialist at 954-450-6888. Some eligibility requirements are: • Fifty-five (55) years of age or older. • Be in need of a protective environment during the day because of a physical, (functional) social and/or mental disability. • A medical form provided by Adult Day Care must be completed by client’s physician and renewed annually. Adult Day Care is supported via numerous funding sources. |
